B.E.S.T Registration Form
Please follow these steps when registering for BEST. Ensure all fields are filled in with the correct information.

1.Check the Summer Courses page for your site/location options with grade levels and times, and jot it down on paper.

2. Type in and select from the menus your registration information and print it out. If filling it in by hand, please do so legibly.

3. Read and sign the waiver form.

4. Send this completed form with payment (payable to BEST, INC) to the address of your site/location manager found below on the form.

NOTE: Payment non-refundable 14 days prior to the beginning date of BEST.

	Read and sign waiver form	WAIVER AND RELEASE STATEMENT MUST SIGN!!!!!
